The Innovative Shared Medical Appointment

It’s time to give as much attention to your heart health as you do your other health issues. Take the first step by scheduling a Heart Secrets Shared Medical Appointment, or SMA.

What’s an SMA?

It is a medical appointment that combines a traditional one-on-one physician visit with a chance to interact with many providers and other women all focused on one topic: women’s heart health. Rather than the typical 15 minute office visit, an SMA is designed to last up to 120 minutes.

An SMA offers many benefits:

• A traditional one-on-one doctor’s visit to discuss your personal health history.
• Participation in a group session with other women and staff from Heart Secrets.
• More time with the physician and other expert providers in hearth health, nutrition and exercise.
• A supportive environment and improved outcomes for you and other women in the program.

During an SMA:

• You’ll be given a global heart risk assessment and lipid profile to learn your risk factors for heart disease as well as your current heart health.
• You’ll have a traditional one-on-one doctor’s visit to discuss your personal health history.
• You’ll participate in a group session with other women and members of the Heart Secrets team.
• You’ll receive valuable information from the Heart Secrets certified nurse practitioner, dietitian and exercise specialist. This is more than the old “eat right and exercise” speech you’ve heard a dozen times; we are here to change the way you think about diet and exercise forever.
• You’ll leave with a personalized plan of care to help you achieve your best health ever.

You can make an appointment for an SMA one of three ways:

• Obtain a referral from your primary physician – some insurance companies will require a physicians’ referral for billing purposes. Perhaps you learned about Heart Secrets from your physician because he or she knows what an effective program this is for women like you!

Payment

The Heart Secrets SMA is a medical appointment and is billed to your insurance company just as any other medical appointment. Call (414) 961-3600 to discuss payment options.
